You can now use the Before Booking custom fields to create your own COVID-19 declaration form. Have customers fill this up each time they make a reservation with you.
Here’s how you can create your own form
Step 1 - Go to Before Booking fields
You can navigate to Before booking fields on dashboard > Settings > Custom Forms > Before Booking
Step 2 - Choose “All services”
Under the “Select a service” field, select All Services as the option. This ensures that all your services have the before booking fields of the COVID-19 declaration form.
Step 3 - Add your questionnaire
In the “Additional fields” section, click on the “Add a field” button to create a new field. Select “Dropdown” for Yes or No questions, or “Text” for asking personal phone number, etc..

NOTE: Please make sure that you check “Is this booking specific” checkbox. This ensures that the form is saved for every single booking.
Step 4 - Save your form & see it in action
After you’ve added all your questions for the declaration, save the form. To see this in action, simply try booking any service on your service store.
And that’s it! That’s how you can add your own COVID-19 Declaration form for all bookings made in your business.
Disclaimer : The fields you add for the COVID-19 declaration form will be appended to any existing fields. You can choose to keep the older fields, or disable them.

- Have you returned from another country or state in the last 14 days? (Yes / No)
- Have you had close contact with or cared for someone diagnosed with COVID-19 within the last 14 days? (Yes / No)
- Have you experienced any cold or flu-like symptoms in the last 14 days (to include fever, cough, sore throat, respiratory illness, difficulty breathing)? (Yes / No)
